 b/c I can't find the original. Amendments are only changes to the main document?​ Yes. If all 
 of these are yes, then I'd probably not include amendment in the Document Type field. 

​Somewhere in here, I also want to add a paragraph field titled "Concerning Survey Answers." 
Probably on the Risk page. It is not a required field.
